Your son/daughter will be involved in a variety of activities, which involve the student being dropped off at a location other than your regular classroom location. School district policy requires the school to have the consent of parents each time the student leaves the school grounds. Rather than send home a permission slip for every activity, we are asking your cooperation in completing this annual permission form, which will cover all local activities where the parent is asked to drop off the student at an alternate location. Further field trip forms will be provided for higher risk activities, activities located outside of the program’s area (Comox Valley or Nanaimo) and for activities that require further transport throughout the school day.
Parents will be informed of any activities at least two school days before they are to take place and all activities will be made available through an online calendar that is accessible to all parents.
Please sign and return this permission form, which would apply to local walking field trips only. One form per student must be completed.
